MRCP II Pastest 2023 Cardiology.
M. Alam
A 61-year-old man is admitted to the Cardiology Clinic after a collapse. He does not recall the circumstances
surrounding the fall, but states that he has been getting chest tightness on exertion lately. This is his third such
collapse in six months. He has a background of previous non-ST elevation myocardial infarction (NSTEMI) and
has an ejection systolic murmur on auscultation.
On examination, his lying blood pressure (BP) is 135/85 mmHg, while his standing (BP) is 133/80 mmHg.
An echocardiogram (ECHO) reveals an ejection fraction of approximately 40%, moderate left ventricular (LV)
systolic function, and moderate aortic stenosis
